Preoperative Preparation

The second appointment we’ll have is your pre-op appointment. It lasts about an hour and takes place about 2 weeks prior to your facelift. 

During this appointment we’ll review the procedure once again, discuss any specific risks, complete all consent forms, conduct a physical exam, and address medical concerns and existing medications like diabetes and blood pressure with lab work if needed. 

We take photos, prescribe medications, and collect payment, while a nurse provides written postoperative care instructions, ensuring you’re prepared without last-minute pharmacy visits.

On the day of surgery, you’ll bring your prescribed medications, which we administer upon check-in. As they take effect I’ll review the surgery again with you, mark the areas, and may supplement oral sedation with an intramuscular injection for comfort during numbing. Local anesthesia allows you to go home promptly post-surgery, fully awake.

Secondary Facelift Surgery Walkthrough

Secondary facelift surgery lasts about 2.5 to 4 hours depending on complexity and scar tissue, begins with numbing or IV sedation, tailored to our agreed preference. 

I may start with fat grafting for facial rejuvenation or SmartLipo for neck contouring to tighten skin and stimulate collagen, enhancing your contours.

Incisions follow your temple hairline, trace natural ear creases, and extend slightly along the hairline, though scar tissue from prior surgery may complicate dissection. I assess your current anatomy, addressing sagging skin, jowls, or neck issues by tightening the SMAS layer or sculpting deeper structures.

For revision cases correcting unsatisfactory prior results (phase two procedures), I focus on specific sequelae, like irregularities. For a secondary tuck, I refine skin tightness after a year’s healing. 

This meticulous approach ensures a natural facelift result tailored to your objectives.

The Recovery Process After Secondary Facelift Surgery

Recovery focuses on healing your incisions to withstand tension, removing stitches after one week to avoid marks, though restrictions persist for 2–3 weeks to strengthen the skin.

I will advise you to avoid lifting anything over 10–15 pounds or activities that elevate your heart rate (such as hills, stairs, biking) to prevent hematomas.

Starting two weeks pre-surgery, you’ll stop blood thinners, and post-surgery, icing and tranexamic acid injections reduce bruising, which may last 1–3 weeks. 

Swelling, more noticeable with facial soft tissue, fades within 1–2 weeks, letting you feel presentable in public. About 80% of your results appear by 1–3 months, with full definition possibly taking longer.

Follow-up includes a day-one visit for wound care training (twice-daily cleaning and ointment), day-three suture removal if needed, and days 7–8 for incision sutures. 

We encourage contact for concerns, offering in-person visits after the first week, with a one-month photo visit and optional check-ins at three, six, or twelve months.

Not everyone is a good fit for this but most of the patients I meet are. I assess your suitability based on medical history, lifestyle, and expectations. Certain health conditions like diabetes, high blood pressure, recent organ transplant, smoking, or unrealistic goals may indicate a poor fit. Of course! Me and my team will see you day 1 for wound care, day 3 for some suture removal, and day 7 for additional suture removal. The one-month visit is especially exciting because this is when we take your after photos. You’ll also have optional check-ins at three, six, or twelve months at your discretion. There is a misconception that you won’t know what type of anesthesia you’ll get and that is completely false. You’ll know exactly the type you’ll receive and it will be tailored to your level of comfort. We offer local with sedation, IV sedation, or general. Most choose oral sedation, but IV sedation ensures unawareness if preferred. Again, it’s all based on what’s safest and what makes you most comfortable.
